The Documents Required with the application are:

1.  Passport Size colour photograph of self ( and add-on applicant if add-on card required).
2. Proof of Income. Form 16A and/or ? Income Tax Return.
3. PAN Card.
4. Proof of Residence in r/o present resedential address (any one of): Photocopy of Passport, Voter ID, Driving Licence, MTNL/ BSNL Land Line Bill.
